Open Enrollment for January

You do not need to take any action unless you are changing networks, adding or removing dependents or switching from the Value plan to HSA plan. If you are only changing your primary clinic-you do that by calling the phone number on your ID card.

The Value Plan is no longer offered as an option for PEIP insurance. You must complete an enrollment form and select a HSA Plan, otherwise you will not have coverage effective January 1, 2025.

If you would like to do any of the following, you must complete enrollment form by 4:00 p.m. on Friday, November 8, 2024. 

• Change plans from Value Plan to HSA Plan

• Change insurance carriers (Blue Cross Blue Shield or HealthPartners)

• Change your coverage significantly, like by adding or removing dependents (Decline, Single, Single +1 or Family)

• Enroll in the HSA (Health Savings Account) if you elected the HSA compatible plan. Note: An HSA account is similar to a savings account for qualified health plan expenses and is provided through vendor Further.

• If you wish to drop your health insurance coverage with the district effective 12/31/24 you must complete the form to decline/waive health coverage.

Clinics may have changed benefit levels and this may affect your deductibles and copayments. Verify with your carrier (Blue Cross or HealthPartners) or access the link here to go to the Innovo web site. The “View 2025 List” link for clinics is on the right side of the page.  Some clinic benefit levels have changed.

Login to the Employee Access Center  and view “deductions and benefits” under the “salary and benefits” button on the left hand side. You will see deduction titles that include HSA, Value or High.
