Osseo Registration Information

The pamphlet containing the following information and forms can be obtained from the Athletic Office (763-391-8600).  All forms must be complete before you will be eligible to play or practice.

  1. General Information: Learn your student I.D.#.  (This ID# follows you from junior high through senior high, if unknown call the school you are currently attending.)
  2. Physical examination: All athletes must have a physical on file in the Athletic Director's office. The physical (history column) form must be updated by the parent/guardian and athlete every year and signed. The physical on file is valid for 3 years from the date of the actual examination signed by the physician.
  3. Mouthguards: Mouthguards are mandatory in Football and Hockey and it is the responsibility of the athlete to provide his/her own mouthguard. Mouthguards are highly recommended in the following sports: Basketball, Wrestling, Soccer, Softball, Baseball, and Volleyball.
  4. Eligibility Information: (pamphlet) This form states the Minnesota State High School League general rules. Please read them carefully. At the end of the pamphlet, there is a two-sided form, which must be filled out and signed on both sides by the athlete and parent/guardian.
  5. Emergency Card: This emergency card must be filled out and signed by a parent/guardian. The coach will carry this card with him/her at all times.
  6. Insurance: This insurance waiver section of the General Information Form must be signed by the athlete and parent/guardian whether you wish to apply for the insurance or not.
  7. Free & Reduced: Any student who wishes to participate in athletics and qualifies for the free and reduced lunch program, must attach a copy of their Letter of Approval from the District Office or a copy of the DHS form from the State of MN.
  8. Financial Hardship: Any student who wishes to participate in athletics, but has a financial hardship should talk to the Building Athletic Director.
  9. *Athletic Fees: The following fees will be charged and collected at the time of registration. Please make checks or money orders payable to Osseo Senior High School. NO CASH.

    All Athletic Fees are $180.00

    *Athletic fees will be refunded if the athlete is cut from the team or quits before the first official contest. Fees may be refunded after the first contest at the discretion of the Athletic Director and coach due to an extenuating circumstance such as a season ending injury early in the season.

After all forms have been turned in, all fees paid and equipment turned in from previous sports, you will be eligible to practice and checkout equipment for your sport.  All of the above forms and fees are to be brought to the Athletic Office at Osseo Senior High School.  It is recommended to have these forms turned in at least three (3) days before your sport begins.
