![]() 1. Medications to be taken at school need a doctor/or medical authorization (including medication, dosage, time(s) to be given and reason). Forms are available in the Health Specialist’s office . It can be faxed to the doctor with a parent ’s signature; the clinic form or copy of prescription are also acceptable. This requirement still includes OTC medications such as Tylenol, Advil , Pamprin or Midol. Cough drops are allowed from home with a parent note.
2. Inhalers also need doctor’s authorization, but students are encouraged to carry and use as needed. It is also suggested that an extra inhaler be provided to health office in case student's inhaler is misplaced or runs out.
3. Please update any emergency contact or phone number changes as soon as possible. We never know when those numbers will be needed , and urgent situations may turn into emergencies if we are unable to contact a parent/guardian when necessary.
