Guidance and Counseling Program Osseo Area Schools guidance and counseling program consists of the following four major areas :
Why would I see my counselor? A student may see a counselor for a variety of reasons. Counselors are trained to help students in four broad areas:
How do I make an appointment? Students may make an individual appointment with their counselor by going to any of the clerical staff in the Guidance Office. Counselors are assigned by alphabet, so a student does not need to know his/her counselor’s name. The clerical staff will then schedule the student with the appropriate counselor. Students are or to inform teachers before they come for appointments so that they can get assignments ahead of time. Parents can make appointments to see their son’s/daughter’s counselor by calling the Guidance Office at 763-391-8700 extension 0.
Registration of a New Student A parent or legal guardian must accompany every student enrolling who is under the age of 18. If accompanied by a guardian, we must have proof of legal guardianship. All of the attached forms must be filled out in their entirety, dated and signed in the appropriate areas. We request that you provide the following information at the time of enrollment. This will ensure proper class placement for your student and the best opportunity for a successful school year at Maple Grove Senior.
_____ DOCUMENTATION OF DATE OF BIRTH —(birth certificate, baptismal record, passport, etc.) _____ IMMUNIZATION RECORD (Health and shot records) Immunizations must be up-to-date. *Students CANNOT start school until this information is received.* _____ TRANSCRIPT from previous school must accompany student at the time of enrollment. (unofficial copy is acceptable.) _____ WITHDRAWAL PAPERS AND WITHDRAWAL GRADES from previous school are required when enrolling after the first day of the school year. _____ SPECIAL EDUCATION RECORDS —if the student is currently receiving services, please provide IEP, Assessment Reports, Psychological Assessments and outside assessments. _____ 504 ACCOMMODATION PLAN —if the student has a plan in place, please provide a copy of the current 504 and medical documentation of the handicap. _____ MINNESOTA GRADUATION STANDARDS TEST RESULTS AND PROFILE OF LEARNING —if the student has taken these tests and/or has completed Profiles, results must be provided. _____ ELL SERVICES —if the student is currently receiving service or needs services, please notify the counselor at the time of your enrollment meeting. _____ PARENT/GUARDIAN SIGNATURE is required on all enrollment forms of students who are not yet eighteen (18). **FREE AND REDUCED PRICE MEALS are available for children from households that meet federal income guidelines. If you would like to apply, please fill out the application in it’s entirety. It is very important to be very thorough when completing this form as any errors or omissions can cause a significant delay in processing your application. If a student wishes to participate in athletics and qualifies for the free and reduced lunch program, a copy of the Letter of Approval from the District Office or a copy of the DHS form from the State of Minnesota must be attached to the application when registering for a sport. Athletic fees will not be waived without a copy of one of these forms.
